This lesson may provide you with proposed solutions and suggestions that can help you to resolve the issues and having a second opinion. 

Before the lesson, it would be more efficient if you can provide the following background for my brief understandings:
  1. What has happened?
  2. What is the impact? Any financial loss?
  3. What is the process discovering the issue? Which departments are involved?
  4. What are the systems involved?
  5. What are the proposed remediation by the departments involved avoiding this to happen again?
  6. Who is handling the issues and monitor the progess of remediation? 
With these information, this can help me to have a brief understanding of the issues and facilitate our discussion.
